Do I Need Imaging for This Pain?

If you are dealing with pain that is not going away, it is very common to ask:

“Do I need an X-ray or MRI for this?”

At our physiotherapy clinic in Collingwood, this is one of the most common questions we hear from active adults.

The short answer is: sometimes – but often, no.

For most muscle and joint pain, imaging does not change the treatment plan and, in some cases, can actually slow recovery.

What Imaging Is Actually Used For

Medical imaging such as X-rays, MRI, and ultrasound can be extremely important in the right situations.

Imaging is useful when we are concerned about:

  • Fractures or bone injury
  • Significant trauma or accidents
  • Infection or systemic illness
  • Tumors
  • Serious or progressive neurological symptoms

When these red flags are present, imaging is absolutely appropriate and necessary.

However, these cases represent a small percentage of people seeking physiotherapy in for pain.

Why Imaging Often Does Not Explain Pain

One of the biggest misconceptions is that pain always means something is “damaged” and needs to be found on a scan.

In reality, research shows that many people without pain have changes on imaging, including:

  • Disc bulges
  • Degenerative joint changes
  • Tendon changes
  • Cartilage wear
  • Rotator Cuff Tears

These findings are often a normal part of aging and adaptation, especially in active people.

When imaging is ordered too early, these findings can:

  • Increase fear around movement
  • Create the belief that the body is fragile
  • Lead to unnecessary rest or avoidance
  • Shift focus away from recovery

Pain is influenced by far more than what a picture can show.

Imaging Does Not Tell Us How to Recover

Even when imaging shows something relevant, it rarely answers the most important questions:

  • Why did this pain start now?
  • What can I safely do?
  • What movements should I rebuild?
  • How do I return to work, sport, or training?

A scan cannot accurately measure:

  • Strength
  • Load tolerance
  • Movement control
  • Confidence
  • Capacity for daily life or sport

These are the things that matter most in physiotherapy and long-term recovery.

When Imaging Might Be Appropriate in Physiotherapy

At our Collingwood physiotherapy clinic, imaging may be recommended if:

  • Pain followed significant trauma
  • Symptoms are worsening instead of improving
  • There is unexplained weakness, numbness, or loss of function
  • Red flag symptoms are present
  • Symptoms do not improve after appropriate rehab

A thorough physiotherapy assessment helps determine whether imaging will actually change management, or whether it is unlikely to add value. If it is recommend then we will fight in your corner to get it organised appropriately!

Why Early Imaging Can Sometimes Slow Recovery

Getting imaging too early can unintentionally:

  • Increase fear about normal findings
  • Shift attention away from movement and function
  • Create a belief that healing is out of your control
  • Lead to passive or overly cautious care

When people believe their body is damaged, they often move less, load less, and recover more slowly.

Good physiotherapy builds confidence and capacity, not fear.

What Should Come Before Imaging

For most non-traumatic pain, the first step should be:

  • A detailed physiotherapy assessment
  • Clear explanation of what is likely happening
  • Education about what is safe vs unsafe
  • A plan to gradually rebuild strength and tolerance

If symptoms improve with movement and progressive loading, imaging was likely unnecessary.

So – Do You Need Imaging for Your Pain?

The honest answer is: maybe – but often no.

Imaging is a tool.

It is not a starting point.

And it is not required for most people to recover.

A better question to ask is:

“What does my body need to tolerate life again?”

That is where good physiotherapy starts.
